OCR found that the practice had failed to conduct an accurate and thorough risk analysis.<\/span><\/p>\n<p><span style=\"font-weight: 400\">And in April 2026, OCR announced four additional ransomware settlements involving breaches affecting more than 427,000 individuals collectively.
